Lets Get Started

First make sure you have Adobe PDF Reader, If your not sure if you have it, you could download it in the first column, It’s the column with the Red icon on the left.

After it downloaded, Open your Downloads Folder Double Click on the Adobe Pdf Reader Icon and Install it.

After that Download the Intake Form on the Left. After you download the Intake form, Right click on it and open with Adobe Acrobat Reader. Fill out the form completely and then click the Submit form at the end of the form. After that a popup will show up, if you don’t use Outlook, choose Gmail or Yahoo put in your email address. It will create the email for you just click send and we will get your form and start the process.
